Hip Pain को नज़रअंदाज़ करना पड़ सकता है भारी 

By Dr. Rajat Jangir, Orthopedic Surgeon in Jaipur बस थोड़ा-सा दर्द है, कुछ दिनों में ठीक हो जाएगा”— यह बात मैं अपनी OPD में अक्सर Hip Pain के मरीजों से सुनता हूँ। लेकिन कई बार यही लापरवाही छोटी-सी तकलीफ़ को बड़ी समस्या में बदल देती है। Hip Pain हर बार गंभीर नहीं होता, लेकिन लंबे समय तक इसे नज़रअंदाज़ करने से स्थिति बिगड़ सकती है और आगे चलकर इलाज भी अधिक जटिल हो सकता है।  Jaipur में अपनी Orthopedic Practice के दौरान मैं अक्सर ऐसे मरीजों को देखता हूँ, जो महीनों या कई बार वर्षों तक Hip Pain के साथ ही रहते हैं और सही समय पर इलाज नहीं करवाते। इस ब्लॉग में हम समझेंगे कि Hip Pain को नज़रअंदाज़ करना क्यों ठीक नहीं है, इसके संभावित कारण क्या हैं और कब आपको Orthopedic Specialist से Consultation लेना चाहिए। Hip Pain को लोग क्यों नज़रअंदाज़ करते हैं?  Hip Pain की शुरुआत अक्सर हल्के दर्द या अकड़न से होती है। सुबह उठते समय जकड़न महसूस होना, लंबी Walk के बाद दर्द बढ़ जाना या बैठने-उठने में हल्की तकलीफ़ होना ऐसे शुरुआती संकेत हैं, जिन्हें लोग अक्सर सामान्य समझकर नज़रअंदाज़ कर देते हैं। कई लोग Painkillers लेकर राहत पाने की कोशिश करते हैं, जबकि कुछ Surgery के डर से डॉक्टर से सलाह लेने में ही देरी कर देते हैं।   सच्चाई यह है कि Hip Pain के ज़्यादातर मामलों को शुरुआती अवस्था में ही पहचान लिया जाए, तो उन्हें बिना Surgery के भी प्रभावी ढंग से Manage किया जा सकता है। इसलिए समय पर Diagnosis और सही Treatment लेना बेहद ज़रूरी है।  Hip Pain के सामान्य कारण   Hip Pain के पीछे कई कारण हो सकते हैं। सही इलाज के लिए सबसे पहले उसके वास्तविक कारण की पहचान करना ज़रूरी है। आइए जानते हैं Hip Pain के प्रमुख कारण।  Hip Pain को नज़रअंदाज़ करने से क्या नुकसान हो सकता है?  1. छोटी समस्या समय के साथ बड़ी बन सकती है  यदि शुरुआत में ही सही Diagnosis हो जाए, तो शुरुआती Osteoarthritis या Avascular Necrosis (AVN) जैसी कई स्थितियों को Medicines, Lifestyle Changes और Physiotherapy की मदद से प्रभावी ढंग से Manage किया जा सकता है। लेकिन लंबे समय तक Hip Pain को नज़रअंदाज़ करने पर Joint का Damage बढ़ता जाता है। ऐसी स्थिति में बाद में Hip Replacement Surgery जैसे उपचार की आवश्यकता पड़ सकती है।  2. चलने का तरीका (Walking Pattern) प्रभावित हो जाता है  Hip Pain के कारण कई लोग बिना महसूस किए अपने चलने का तरीका बदल लेते हैं। शुरुआत में इससे दर्द थोड़ा कम लग सकता है, लेकिन धीरे-धीरे इसका अतिरिक्त दबाव Knee, Lower Back और दूसरे Hip Joint पर पड़ने लगता है। समय के साथ इससे अन्य Joints में भी दर्द और परेशानी शुरू हो सकती है।  3. Hip Pain का असर आपकी Daily Life पर पड़ सकता है  यदि Hip Pain को लंबे समय तक अनदेखा किया जाए, तो धीरे-धीरे सामान्य काम भी चुनौतीपूर्ण लगने लगते हैं। सीढ़ियाँ चढ़ना, ज़मीन पर बैठना या लंबी Drive करना तक मुश्किल हो सकता है। इसके कारण कई लोग अपनी Physical Activities कम कर देते हैं, जिससे Weight बढ़ने लगता है, Muscles कमज़ोर होने लगती हैं और Overall Fitness भी प्रभावित होती है।   4. Mental और Emotional Health पर असर   लंबे समय तक रहने वाला Hip Pain सिर्फ़ शरीर तक सीमित नहीं रहता। यह आपकी नींद, मनोदशा (Mood) और रोज़मर्रा की गतिविधियों को भी प्रभावित कर सकता है। लगातार दर्द के कारण चिड़चिड़ापन बढ़ सकता है और सामान्य कामों के लिए दूसरों पर निर्भरता भी बढ़ने लगती है।  5. ACL इंजरी के बाद इन गलतियों से बचें – वरना Recovery मुश्किल हो सकती है। 

By Dr. Rajat Jangir, Orthopedic Surgeon in Jaipur अगर खेलते समय अचानक घुटना मुड़ जाए, “Pop” जैसी आवाज़ सुनाई दे और कुछ ही समय में घुटने में दर्द व सूजन आ जाए, तो इसे सामान्य Sprain समझकर नज़रअंदाज़ न करें। यह ACL Injury का शुरुआती संकेत हो सकता है।  ACL (Anterior Cruciate Ligament) घुटने का एक बेहद महत्वपूर्ण Ligament होता है, जो Femur (Thigh Bone) और Tibia (Shin Bone) को स्थिर (Stable) रखने में मदद करता है। इस Ligament के Tear होने पर केवल दर्द ही नहीं होता, बल्कि चलने, दौड़ने और Sports Activities के दौरान घुटना बार-बार अस्थिर (Unstable) महसूस हो सकता है।  अपनी Orthopedic Practice के दौरान मैंने देखा है कि कई मरीज Injury से ज़्यादा, उसके बाद की गई गलतियों की वजह से परेशान होते हैं। आइए जानते हैं कि ACL Injury के बाद किन गलतियों से बचना चाहिए। ACL Injury कैसे होती है? जानिए इसके मुख्य कारण।  ACL Tear आमतौर पर Football, Cricket, Badminton, Basketball और Running जैसे Sports के दौरान होता है। अचानक Direction बदलना, Jump के बाद गलत तरीके से Landing करना या घुटने पर तेज़ Twist आना ACL Ligament Tear के प्रमुख कारणों में शामिल हैं।  Road Accident, सीढ़ियों से गिरने या घुटने पर ज़ोरदार चोट लगने के बाद भी ACL Injury हो सकती है। ऐसे में कई लोग इसे केवल मोच समझकर नज़रअंदाज़ कर देते हैं या घर पर ही इलाज करते रहते हैं, जिससे Injury और गंभीर हो सकती है।  1. दर्द कम होते ही सब कुछ Normal मान लेना  ACL Injury के बाद शुरुआती कुछ दिनों में दर्द और सूजन कम हो सकते हैं। इसी वजह से कई लोगों को लगता है कि Ligament पूरी तरह ठीक हो गया है, जबकि ऐसा ज़रूरी नहीं होता।  हकीकत यह है कि Complete ACL Tear अपने आप ठीक नहीं होता। अगर Ligament क्षतिग्रस्त (Damaged) है और समय पर उसका उचित इलाज नहीं किया जाता, तो घुटना बार-बार फिसलने (Giving Way) या अस्थिर महसूस होने लग सकता है। समय के साथ Meniscus और Cartilage को भी नुकसान पहुंच सकता है, जिससे भविष्य में Arthritis का खतरा बढ़ जाता है।  2. सिर्फ़ X-ray पर भरोसा करना  Knee Injury के बाद केवल X-ray करवाना पर्याप्त नहीं होता। X-ray से हड्डियों (Bones) की स्थिति का पता चलता है, लेकिन Ligaments की स्थिति का सही आकलन नहीं हो पाता।   अगर डॉक्टर को ACL Tear का संदेह हो, तो MRI करवाना ज़रूरी होता है। Clinical Examination और MRI की मदद से Injury की गंभीरता (Severity) का सही आकलन किया जाता है, जिसके आधार पर उचित Treatment तय किया जाता है।  3. ज़रूरत होने के बावजूद Surgery में देरी करना  हर ACL Injury का इलाज Surgery से नहीं किया जाता। हालांकि, यदि Complete ACL Tear के कारण घुटना बार-बार Giving Way करे या अस्थिर महसूस हो, तो ACL Reconstruction Surgery बेहतर उपचार विकल्प हो सकती है।  कई मरीज Surgery में अनावश्यक देरी कर देते हैं। इस दौरान घुटने की बार-बार होने वाली अस्थिरता (Instability) के कारण Meniscus और Cartilage को लगातार नुकसान पहुंच सकता है। समय के साथ यह Damage स्थायी हो सकता है और आगे चलकर Arthritis होने का जोखिम बढ़ा सकता है।  4. Physiotherapy को गंभीरता से न लेना  ACL Injury के बाद चाहे Surgery करवाई गई हो या Conservative Treatment अपनाया गया हो, बेहतर Recovery के लिए Physiotherapy बेहद ज़रूरी होती है।  केवल Rest करना ACL Injury से पूरी तरह ठीक होने के लिए पर्याप्त नहीं है। बेहतर Recovery के लिए Structured Rehabilitation और नियमित Physiotherapy बेहद ज़रूरी है। इससे घुटने की Movement में सुधार होता है, Muscles दोबारा मज़बूत बनती हैं, Knee की Stability बढ़ती है और भविष्य में दोबारा Injury होने का जोखिम कम होता है। इसलिए Physiotherapy को बीच में छोड़ने या Skip करने की गलती न करें।   5. बहुत जल्दी Sports या Gym शुरू कर देना  दर्द कम होते ही कई लोग Running, Football या Gym दोबारा शुरू कर देते हैं। लेकिन जल्दबाज़ी में लिया गया यह फैसला Recovery को धीमा कर सकता है और दोबारा Injury का खतरा बढ़ा सकता है।  ACL Reconstruction के बाद नए Graft को पूरी तरह मज़बूत होने में आमतौर पर 9–12 महीने लगते हैं। इस अवधि में जल्दबाज़ी में Sports, Running या High-Impact Activities शुरू करना सही नहीं है, क्योंकि इससे Graft पर अतिरिक्त दबाव पड़ सकता है। बेहतर Recovery और सुरक्षित Return to Sports के लिए हमेशा डॉक्टर और Physiotherapist की सलाह का पालन करें।  6. वजन और Muscle Strength पर ध्यान न देना   अधिक Body Weight घुटनों पर अतिरिक्त दबाव डालता है। इसके साथ ही, यदि Quadriceps और Hamstring Muscles मज़बूत न हों, तो घुटने की Stability कम हो जाती है और Recovery में भी रुकावट आ सकती है।  बेहतर Recovery के लिए Balanced Diet, Weight Management और नियमित Strengthening Exercises बेहद ज़रूरी हैं। ये Muscles की ताकत बढ़ाते हैं, घुटने को बेहतर Support देते हैं और सामान्य गतिविधियों में सुरक्षित वापसी में मदद करते हैं।  7. ज़रूरत पड़ने पर Second Opinion न लेना  हर ACL Injury की गंभीरता अलग-अलग होती है। कुछ मरीजों में Partial Tear होता है, जिसका इलाज Rehabilitation से किया जा सकता है, जबकि Complete ACL Tear की स्थिति में Surgery बेहतर उपचार विकल्प हो सकती है।   यदि आपको Treatment Plan या Surgery को लेकर कोई संदेह हो, तो अनुभवी Orthopedic Surgeon से Second Opinion लेने में संकोच न करें। सही सलाह आपको अपनी स्थिति के अनुसार बेहतर निर्णय लेने में मदद करती है।  ACL Injury के बाद क्या करना चाहिए?  बेहतर Recovery के लिए इन ज़रूरी बातों का पालन करें: ACL Injury से बचने के टिप्स  Conclusion  ACL Injury को नज़रअंदाज़ करना भविष्य में घुटने की बड़ी समस्या का कारण बन सकता है। यदि समय पर Diagnosis हो, सही Treatment लिया जाए और Rehabilitation पूरी ईमानदारी से की जाए, तो अधिकांश मरीज फिर से अपनी सामान्य दिनचर्या और Sports Activities में सुरक्षित वापसी कर सकते हैं।  अगर Knee Injury के बाद दर्द, सूजन या घुटना बार-बार अस्थिर (Unstable) महसूस हो रहा है, तो इसे नज़रअंदाज़ न करें। बिना देर किए Orthopedic Specialist से सलाह लें। समय पर Diagnosis और सही Treatment बेहतर Recovery के साथ-साथ भविष्य की जटिलताओं के जोखिम
